CLICK Take quiz

When complete:CLICK Submit (the computer will score your responses).

Note:

1.     You must answer at least 10 questions to successfully complete the refresher training. EH&S will then issue certification by E-mail & campus mail. Contact EH&S if you have not received the training documentation within one week.

2.     If you FAIL to successfully complete the quiz, you must contact EH&S (7-4712).

3.     Remember your registration is good for only one week. If you fail to take the required quiz by the closing date, your lab will be considered in non-compliance with this Federally mandated regulation.
