MINOR AND SUMMER STUDENTS

Medical Center departments and individual Principal Investigators, are encouraged to permit minors and other students to work in research laboratories during the summer months. The linked forms must be completed by Principal Investigators or Authorized Users who intend to employ these students. All requested information must be provided on these forms and submitted to Environmental Health & Safety (EH&S) as soon as possible.

The forms are fillable using Adobe Acrobat Reader, but you will not be able to save them to your computer unless you have the full version of Acrobat. The Parental Consent must be printed for signature.

Upon receipt of the completed form, we will schedule the laboratory safety training classes (including biological, chemical and radioactive materials). The scheduled training programs provide general safety training, however PI’s are required to further supplement this with the required laboratory specific training.

In accordance with GU policy and DC regulations, minor students must receive written parental consent prior to participating in any research activity. Please remember to indicate which hazardous materials are present in your laboratory prior to providing the form to the minor for parental consent. A copy of the completed form (with parental signature), must be forwarded to both EH&S and Human Resources.
