Conflict of Interest Disclosure Study Specific Disclosure Form is Required for All Proposal Submission ALL Georgetown University investigators submitting proposals to an external sponsor are required to complete a Study Specific Disclosure Form [PDF] [MS Word] and submit a signed copy either to the Office of Sponsored Programs (Main Campus) or the Office of Sponsored Research (Medical Center) as part of the proposal review process. Investigators include the principal investigator and any other person who is responsible for the design, conduct or reporting of research.   For additional information, please read the memo.
